I submit this application freely and willingly as a gesture of my interest in applying for employment. I declare the information I provided herein is true, complete and represents an accurate statement of facts. I may make corrections to the information included herein in writing if needed within a reasonable time. If hired, I agree to abide by all workplace policies whether written, verbal or otherwise as they may exist today and as they are expected to change from time to time during my term of employment. I understand any intentional misrepresentation or omission of facts in this application or made by me during my term of employment is cause for dismissal. I also acknowledge this application is not to be considered an employment contract. If hired, I acknowledge I will be employed on an “at will” basis during my entire term of employment and either I or my employer will have the right to end our employment relationship at any time with or without notice or for any or no reason whatsoever. I understand any offer of employment or continued employment is not guaranteed for any length of time. No one, except the owner, within a separate written employment contract agreement, may change the “at will” aspect of employment. I authorize an investigation of all statements contained in this application now and during the course of my employment. I hereby give the Employer or their agents or assigns permission to verify the information I submitted herein. In addition, they may make a determination of my suitability for employment by conducting applicable motor vehicle, criminal background checks, or other local, state and/or federal investigations as well as contact my schools, my previous employers (unless otherwise indicated), and personal references for the purpose. The employer shall also consider my level of adherence to societal, legal, ethical, moral and employment standards when determining my eligibility to continue in the interview process, if selected for an interview. I release the employer and their agents or assigns from any liability as a result of such contact or acting upon these statements of references on my behalf. In some cases, I may be required to sign an additional authorization form in order for the employer or their agent to initiate background checks on my behalf as mandated by local, state or federal law.
